DECLARATION OF HONORS FORM

This form is to be used by students to declare their intention to pursue a University Honors degree.
Complete this form and return it to the Honors Coordinator of the college whose requirements you’ll
be completing to earn the University Honors diploma (usually your home college). This form
should be submitted before the end of your sophomore year.

Enter below those components of the Honors Program you have already completed, and indicate
which components you are planning to pursue to complete the required 18 credits. The form needs
to be signed by your academic advisor and by the College Honors Coordinator. Submit the
completed signed form to the appropriate College Honors Coordinator with copies to your advisor
and the College Evaluator of that college.
