The 2009 Form W-4 is available for download as an Adobe .pdf file at the bottom of this page and in the Payroll Department. It is also available from the IRS here*

Employees who claimed exempt status in 2008 and wish to continue their exemption for 2008 must submit a new Form W-4 to the payroll department by 02/13/2009. These employees should complete only lines 1, 2, 3, 4, and 7 and sign the form to validate it.
If an employee whose exemption expires on February 14th does not file a new form, withholding will be as if the employee were single, claiming no withholding allowances until the employee submits a new Form W-4 .
