welcome to download any of the International Center's forms for your use. Please be advised
that many of these forms require the signature of the University's International
Student Advisor (GSU 327) and are not valid with out it.

Status Verification
This letter is intended to verify your attendance at the University of
Hartford. It may be used for Motor Vehicle, State ID, Social Security
or other reasons.
You may complete and
download this letter and return it to the International Center for an
authorization signature and stamp.

Transfer-in School Form
Use this form if you are
transferring from another institution TO the University of Hartford
Transfer-out School Form
Use this form if you are transferring FROM University of
Hartford to another university.

Part-time Enrollment
Verification
Current University procedures and CIS SEVIS regulations require that a
student’s faculty advisor or other school official verify the reason a
student must be enrolled part-time. Permission to enroll part-time may only
be granted on a per-semester basis and must be accompanied with an
explanation for the request. Part-time status may ONLY be legally (through
the government SEVIS system) approved
by the International Student Advisor. |

Change of Address Request
Form
International Students must notify the Citizenship and Immigration Service of any change of
their local address within 10 days. Please return this form to the
International Center, GSU, room 327 for processing. Your new address(s)
will be placed in the SEVIS system and the University of Hartford Banner
system.

Guarantor Statement of
Support
This form is required as
part of your admission application. You will not be issued an I-20 or DS 2019
form unless this statement has been completed and supporting financial
documents attached, submitted and approved by the International
Center.

Letter
You may use this form to
invite parents, relatives or friend to visit with you while you are studying
at the University of Hartford. This form must be signed by the International
Student Advisor.

Immunization Form
This form is required to
be completed and submitted to Health Services at the time of your course
registration. You may be prevented from registering in the future without
completing your health records. This is required by the University and
the State of Connecticut.

Change of Address Form
Use this form to notify the
Citizenship and Immigration Service of your change of local address in the
U.S. It is best to change your address by simply notifying the International
Center within 10 days of the change. The Center will in turn notify
the CIS through the SEVIS System of the change for you.

Social Security Forms

Social Security Application
Use this form to apply for
your Social Security number. When applying for a number you must bring
your passport, I-20 or DS 2029 form, I-94 Arrival/departure Record (3X5
white card in your passport), a piece of mail showing your local address and
a letter from your on-campus employer and a letter from the International
Center stating your full-time enrollment in courses.
