Faculty Senate Curriculum Committee Form Submission Sequence |
| 1) The requesting college / school / division / department obtains the current version of the proper form. |

| 2) The forms are completed and signed within the College by the Department Chair, the College Curriculum Committee, and the Dean. |
 |
| 3) The forms with all attachments are delivered to Nancy Philip in the Registrar's Office, Computer Center by 9:00AM on the specified due date. |

| 4) The Registrar's Office records the submission and forwards a copy of all materials to the Secretary of the Faculty Senate, Maria Marques, who duplicates the materials and distributes it to committee members. |
 |
| 5) On its scheduled meeting date the Faculty Senate Curriculum Committee convenes, reviews the request, and generates a recommendation. |
 |
| 6) At the subsequent scheduled Faculty Senate meeting the Chair of the Curriculum Committee presents the recommendation to the Senate for their action. |
