Faculty Senate Curriculum Committee Form Submission Sequence

1) The requesting college / school / division / department obtains the current version of the proper form.
2) The forms are completed and signed within the College by the Department Chair, the College Curriculum Committee, and the Dean.
3) The forms with all attachments are delivered to Nancy Philip in the Registrar's Office, Computer Center by 9:00AM on the specified due date.
4) The Registrar's Office records the submission and forwards a copy of all materials to the Secretary of the Faculty Senate, Maria Marques, who duplicates the materials and distributes it to committee members.
5) On its scheduled meeting date the Faculty Senate Curriculum Committee convenes, reviews the request, and generates a recommendation.
6) At the subsequent scheduled Faculty Senate meeting the Chair of the Curriculum Committee presents the recommendation to the Senate for their action.

As there are multiple steps in the process and a number of detailed forms to be completed it is important to be aware of possible 'trip points'. Listed below are some of the items that have caused difficulty in the past.
