This form is to be used by College of Arts and Sciences Students to declare their intention to pursue an Honors degree. A summary of the Academic Honors Program and a list of requirements can be found through the link on the main menu, it is labeled curricular requirements.

Students should enter below those components of the Honors Program they have already completed, and should indicate which they will probably pursue in the future. The form should be signed by the advisor, and by the College's representative to the University Honors Commitee, Professor Jilda Aliotta, or in her abscence, by the Associate Dean. Students should submit the signed form to their department Chair, with copies to the advisor, Professor Aliotta, and the Associate Dean.
